- What tools will I use to complete my responsibilities as a mini-grant recipient?
1. School Health Index: an online self-assessment and planning tool that schools can use to improve their health and safety policies and programs
- What are the deadlines for completing the School Health Index?
Grant Year 1:
By April 30, 2013 you should submit a module summary report for modules 1, 3, and 4.
By April 30, 2013 you should submit an overall scorecard summary report.
By May 31, 2013 you should submit your Plan for Improvement/Action Plan based on the School Health index findings and your approved program.
Grant Year 2:
A module summary report for modules 1, 3, and 4 should be submitted by May 31, 2014.
An overall scorecard summary report should be submitted by May 31, 2014.
All reports should be emailed to the grant manager.
- Where can I find the recorded webinar series?
The recorded webinar series created by the mini-grants team can be accessed through the Resources page.
- What are satisfaction surveys and where can I find them?
There will be two required satisfaction surveys, one in May 2013 and one in May 2014. These surveys provide valuable information to our mini-grants team in regards to how our award recipients feel about the processes required, resources provided, and technical support available. More information about these surveys will be shared closer to distribution.
- How do I get reimbursed for expenses?
Awardees will have two opportunities for reimbursement.
For reimbursement of all related expenses that have occurred by May 31, 2013, submit an invoice on official letterhead and all related receipt documentation. Documentation received after June 30th, 2013 cannot be reimbursed.
Submit an invoice on official letterhead with all related receipt documentation for reimbursement of the balance of the total awarded amount no later than May 31, 2014. Documentation received after June 30th, 2014 cannot be reimbursed.

- Where do I enter my data?
Each school will be required to create a team user account for the Online School Health Index system.

In addition, the four-part webinar series created by the PRO Wellness Center mini-grants team provides an overview of the Online School Health Index system. This series can also be accessed through the Resources page.
These mini-grants are delivered by Penn State Hershey PRO Wellness Center. Funding is provided by the Pennsylvania Department of Health through the Preventive Health and Health Services Block Grant from the Centers for Disease Control and Prevention.
